18F-FDG Cardiac PET/CT (Myocardial Viability Scan)

Patient Guidelines

PREPARATION:      

On the eve of the exam:

  • At dinner, DO NOT EAT CARBOHYDRATES and follow a diet as shown in the table below;
  • Do not perform physical exercises;
  • Maintain continuous medication (except insulin);
  • Bring the study of recent myocardial perfusion. The analysis of the viability study is compared to the perfusion study;
  • This test is not recommended for pregnant and breastfeeding women.

 

Foodstuffs that may be eaten on the day before the exam (allowed)
Eggs, cheeses, meats, sausage, ham, fish and chicken (boiled, fried or grilled).
Vegetables: lettuce, cucumber, tomato, chayote, zucchini, cauliflower, kale, broccoli, watercress, escarole (chicory), chicory, arugula, catalonia, celery, gilo.
Seasonings: salt, garlic, oil, olive oil, margarine, parsley and chives, onion, coriander, pepper.
SUGAR-FREE liquids: water, diet soda, tea.
Sweeteners
Diet gelatin.

 

On the day of the exam:

  • Fasting for 6 hours, except for water;
  • Bring the list of medications on the day of the exam. For patients who use insulin daily, they should not use it at home on the day of the exam;
  • Keep well wrapped, if the outside temperature is low, including on the way to the clinic;
  • Drink plenty of water (at least 3 glasses) before the exam, and you can go to the bathroom normally.

 

HOW IS THE EXAM PERFORMED?    

After filling out the registration form at the reception desk, the patient will undergo a clinical interview.

Then, they will be sent to the preparation room where capillary glucose will be measured.

In general, the most commonly used protocol for non-diabetic patients is oral glucose overload with intravenous insulin supplementation, if necessary. Glucose overload transiently elevates blood glucose and stimulates the release of insulin by pancreatic cells. This causes a reduction in the use of fatty acids by the myocardium and an increase in the use of glucose.

After checking blood glucose, dextrose + water will be provided. Then, serial blood glucose measurements will be performed to the optimal range for 18F-FDG injection.

If there is no ideal blood glucose range or in uncontrolled diabetic patients, intravenous insulin should be administered, making the process a little longer, in this case, the exam time can reach up to 6 hours.

After injection of 18F-FDG, the patient should be kept at rest for 45 to 60 minutes. Diabetic patients should wait 60 to 90 minutes.

Then, the patient will be sent to the exam room. During the entire time of the test, the patient must remain motionless, lying on the device’s gurney.

The exam will begin with computed tomography, a phase of rapid duration. Subsequently, the acquisition of PET begins, which lasts around 10 to 15 minutes.

 

IMPORTANT NOTES:

Eat well after the exam is released;

Abundant hydration is recommended.
